We are a concierge style family medical practice offering both traditional and functional treatment approaches to health care. Your care is personalized and tailored to meet your specific health care goals and needs.  

Choosing a concierge health care provider like Elevate Health & Wellness offers you services not normally provided through traditional clinics.  In person, new patient, appointments are 60-90 minutes in length versus the typical 10-15 minutes experienced at other clinics. More time ensures your needs are addressed and helps build a strong patient-provider relationship.

As a concierge provider we are a cash pay per visit. Insurance is not accepted. 
Superbills will be provided upon request for insurance reimbursement.
